Cracked tooth: Your tooth likely has a fracture and needs a crown! count on it!

Pulpitis: Many possible reasons. Ask your dentist for a referral to an endodontist to help determine the pulpal status. If there is a crack it can often be visualized under high-power operating microscope. Unfortunately from what you are describing the tooth may need a root canal.

Repeated fillings?: Just curious how long the time frame was from when the tooth was first filled to the last? If it was all within a short time frame, something doesn't make sense. This should not be. Possibilities are choosing the wrong restoration to restore the tooth, some other problem not addressed, failing to follow instructions or care of filling\tooth or poor quality of dentistry. Seek a 2nd opinion.
