Both are lousy: Get back to your doctor for treatment that may include immobilization and bone stimulation, possibly nutritional supplementation as well.

All avn is not good: Avascular necrosis means loss of blood supply to that area. Patchy or diffuse doesn't matter, except that patchy might indicate that blood supply is starting to return to the area. When the bone doesn't get the blood supply, the bone starts to demineralize making it appear "patchy" at first. If blood supply is not restored, the calcium in the bone will continue to be absorbed and collapse.
