Neck or Thoracic: Outlet are like places. Nerve conduction tests might help locate the site. Consider seeing your pcp to arrange this or consider seeing a neurologist or neurosurgeon.

Look to the neck: Agreed that the neck is a more likely source of nerve involvement. The lower nerve roots coming out of the neck do involve the pectoralis, and the armpit area. If PT is not taking care of the problem, seek specialist evaluation and likely cervical (neck) MRI which can include the upper few thoracic mid back segments!
