Yes: Open is best, needle will not get good or complete info. But most are benign and if tracked with imaging will not need biopsy.

YES: Most do not require biopsy. Radiologic characteristics will provide basis/indication for more invasive evaluation such as biopsy. Yes, but: Calcification of a lung nodule frequently indicates that it is benign in nature. It is the noncalcified nodules that are most concerning for lung cancer. And thus the noncalcified nodules that more frequently meet criteria that prompts biopsy of the nodule. Appropriate treatment of lung nodules is best done at facilities with experience
