Not typically: If the pain is running from your lower back down your leg i would be more worried about something like sciatica where the nerve e down the back of your leg is pinched or trapped or inflamed leading to those symptoms. If the pain is running from your anus or near there down your leg than i could see how bad perianal pain might feel that way. Best to see an md to help u differentiate.

Not piles: What you describe is not from piles, but from back issues. Piles can be evaluated and treated by a general surgeon, or colo-rectal surgeon, often in the office. But what you have is another issue- and since you are having sciatica with it would recommend you see your physician and physical therapist for some relief. Good luck- miserable thing both piles and back issues.

Piles, leg pain?: The real reason for your pain would be different. Most people with piles are constipated, which means they strain at stools. This creates increased pressure within the spine. If you already have a slightly bulging disc in the lumbar spine and are on the verge of pinching the nerve root, increased pressure while straining can pinch this nerve root and cause the sciatica like pain going down the leg.
