Stage IV ovarian ca: Unfortunately, it is likely that she has stage IV ovarian cancer with spread to the liver. Nothing else would give a ca125 of 5000. She should be on biopsied for a diagnosis and be on systemic chemotherapy which can be very helpful in selected cases.

Depends on imaging: An answer to that question can only be determined by reviewing the actual images (staging requires visualizing at least the abdomen and pelvis). An abdomen ct can be submitted to http://conciergeradiologist.Com in the next month for an assessment that would better be able to answer your question.

It is likely a cance: A high ca 125 leads us to look for ovarian cancer as the most likely cause(sometimes it is due to primary perotoneal cancinoma or fallopean tube tumor). The ct scan may show some intraperitoneal disease or pelvic disease including ovarian massess. But she needs a biopsy proof of cancer and then will likely require treatment for ovarian cancer which often includes debulking surgery and chemotherapy.
