Yes: Absolutely. A heart attack can present with hr and BP changes, but it does not have to be.

Left arm pain 6 to 8 hours, nausea, high BP & pulse (normal 100/55 & 45 went to 125~180/65~90 & 55). Went to ER, bloodwork and EKG indicated no heart attack. 3 days later rode bicycle 17 miles, no problems (rode 4900 miles in 2020). Feeling so-so.
