Vomiting is: A significant problem, because you can get dehydrated rapidly. I don't think nausea, vomiting, or headaches are related to your hiatal hernia. Get medical help immediately.

Maybe not gut: Nausea, vertigo, head pain more likely not gut related. Get inner ear, neurologic exam. May be sinus, etc.
