Maybe: Not all cuff tears absolutely require surgery; however if completely torn it will not heal by itself leaving the patient week in certain motions.

See below: This depends on the extent of the tear in the rotator cuff. Small ones can be left untreated as long as they do not affect the ability to move or use the arm. A larger tear may need to be surgically treated if pain or impairment is noted. Talk to a surgeon, and if needed get a second opinion before reaching a decision.
