Probably surgery: If by advanced you mean symptoms (pain, numbness, tingling, and/or weakness) are severe enough to interfere with normal activities, surgery to take pressure off the nerves (decompression) is more effective than nonsurgical treatments.

One that helps: Thay can vary from patient to patient depending on age & health status as well as associated spinal diagnoses in addition to stenosis. Also depends on the level of your symptoms & findings irregardless of imaging results unless there are cord changes . Smoking , neuropathy & diabetes affect results. Best start with: meds, exercise/pt, consider epidural steroid injection, back support 0 lordosis.
