Pain in Feet: Has he had a recent x-ray and tested for ra? I would consult with his doctor on this and have some blood tests and possible mri.

Progressive: Arthritis can be caused from several other disorders. To get a better idea why it's feels like it's spreading, he may need to get a referral from his pcp to see a rheumatologist and get lab work done. He may also need imaging of his body to see how progressive the arthritis is.
