Hammertoe: Sometimes a long 2nd toe can be squeezed too often in tighter shoes. If our 2nd toe is longer than the great toe, t can be a problem fitting into proper supportive shoes. This could lead to a hammertoe and could lead to damage to the joint, furthering damage to the nerves through swelling and deformity.

Nerve compression: Depending on the heel height, your foot can slide forward even in a wedge and in some cases can keep pressure at the top/tip of the 2nd toe. This pressure can create nerve compression and cause numbness. To alleviate this you can try a lower heel or consider use of a ball of the foot pad just behind the ball of the foot to restore the transverse arch. You can also see a podiatrist for options.
