See surgeon now: Having new swelling and discomfort 3 weeks after any surgery is a sign that there is likely an infection. You need to see your periodontist or surgeon who did the graft asap.

Possible infection: While swelling and pain 7-10 days after a surgical procedure are to be expected, to be present 3 weeks later suggests an infection. While hematomas generally appear shortly after a surgical procedure, not usually much later, they can become infected as well. See your surgeon as soon as possible for evaluation and possible treatment. Delay could compromise the success of your graft.

Yes: It can happen that graft placed under the existing gum tissue with alloderm or xenograft will cause some swelling which will last about three days. Now if the periodontist used an alloderm material such as alloderm, the response will be much less in general. Next time ask your periodontist to use a donor material.

Too long: We would not expect swelling after such a long time. It comes and goes early on. Call the surgeon and tell him/ her you situation. You need to be seen.
