You don't need one : If you have never been sexually active you do not need an internal exam unless you are having a concern, like pain, discharge, bleeding problems.

No short answer: If the practitioner is told of your concerns, they will usually use smaller instruments than in non-virgins, take extra care not to rush during the exam, and some will add the use of a mild anesthetic jelly. Not all practitioners have these supplies or equipment, however. A gynecologist's office is the best bet for the most comfortable exam.
