Malathion lotion is usually the next step if 2 treatments with permethrin haven't helped. Sulfur applications and ivermectin given orally are also other advanced treatments. I think it would be best to consult a dermatologist, experienced in treating resistant scabies, for accurate diagnosis and appropriate treatment first. You may have to consult one at a university hospital near you.
