As a pathologist, I would be more comfortable with the approach of the plastic surgeon. Neither the dermatologist nor the plastic surgeon can determine if it is an intradermal nevus or a melanoma with 100% certainty. This requires biopsy and a pathological examination, When the skin is cauterized, it makes it difficult to "read" so if there was a lesion on the resection margin, it can be missed.
