As an approximation, probably 80-90% of secondary syphilis cases include skin rash. Absence of rash in someone at risk for secondary syphilis (on the basis of exposure history or other symptoms) reduces the likelihood of secondary syphilis, but certainly does not eliminate the possibility. If there is any risk or concern for syphilis, blood tests always are necessary whether or not there is a rash
