You likely have primary hyperparathyroidism again. Unfortunately, you forgot to tell us how your serum calcium level is. High serum calcium could cause all your symptoms due to calcified pericapsular tissue, pseudogout and worsening bone structure because calcium is taken away from all bones due to high PTH activity.
