A complex ovarian cyst can have multiple adjacent sacs separated by tissue walls (septations). Picture a peeled orange, where each slice is separated by its own skin. These cysts are commonly filled with fluid. The thicker the fluid (blood or pus are just some examples), the more it produces echoes on ultrasound. Watery fluid like serum tends to not be echoic.
