Ill-defined hypoechoic focus in the left ovary measuring 0.9 x 0.9 x 0.9 cm with internal echoes. there is no definite posterior acoustic enhancement to suggest a cyst. there is normal doppler flow to the left ovary. what does this mean 60 year old?

It could still possibly be a hemorrhagic cyst in ovary, which would have internal echoes and are not uncommon. They should follow it up somehow to determine it's not a small "mass". I can't say it's definitely not cancer, because it is still possible, but I think the odds are in your favor it's likely something less serious.
