Start with a general practitioner that will be able to further coordinate if consults needed. Losing weight becomes critical and this is where a primary care doc will be able to help.

I would start with a a general physician. With a BMI of 47 I would measure BP at your wrist as arm too big for most cuffs which will lead to false high readings. My best success with DASH diet and weight loss is 150 lbs. And best proven way to lower BP so you may not need meds.
