It is mild deficiency, not quite serious but you definitely need B12 supplement. Although the lower limit of normal is around 200 but most people have level much higher than that and we often advise B12 supplement if there is a suspicion of B12 deficiency by clinical history, symptom or sign although the level is not below 200 yet.
