Neurology to still do an evaluation and consider MRI/MRA to rule out brain structural abnormalities contributing to symptoms. Acoustic neuroma, blockages in the circulation to the brainstem, and possible other factors are not excluded by ENT evaluation alone. Anxiety sounds very severe. Check blood work to rule out thyroid or adrenal gland problems when that severe. Good luck, get evaluated.
