I assume your father has severe ischemic cardiomyopathy. His abnormal kidney function is purely pre-renal azotemia, i.e. renal insufficiency is due to heart failure with impaired renal perfusion. The only way to improve the kidney function (for lowering BUN & creatinine ) is the treatment to improve left ventricular ejection fraction. I am not sure if ESMR is making any difference.
