Supraclavicular lymph nodes (right above the collar bones) drain directly from the chest/abdomen/arms. The first step is to determine if the nodule is truly a lymph node or something else like a benign lipoma or sebaceous cyst. If it is an abnormally large lymph node, a fine needle aspiration biopsy may be needed. Otherwise, a large lymph node may indicate pathology from the chest/abdomen/arms.

Not due to the "node" that has not changed in 4 years. You may feel the node every couple of weeks, if the lymph glands doubles in size, becomes painful, ulcerates, or other nodes become enlarged, or you develop fever, or lose weight, you should see a doctor. Wish you good health!
