Removing a well fitting crown is not necessary. A hole can be drilled through the top for access to the pulp chamber and canals. If the margins are starting to break down after seven years it's time for a new crown, too.

If there is recurrent decay the crown will probably have to be removed, the decay eliminated, and a new crown fabricated, in addition to probable root canal treatment. If there is no sign of decay, entrance to the tooth's root canal system can be made directly through the existing crown.
