My son stepped on something, ankle rolled, heard a snap. er told him the swelling made it so they couldnt see if there was an ankle fracture. is this a thing?

Fractures are assessed by using x-rays, not just an exam where a sprain can look as bad. You didn't give the age of the kid and the growth plates of the long bones can make assessing ankle fractures difficult, even with x-rays. Without an x-ray at any age I could not properly assess a kid for this.
