After a 60 + male has knee surgery and is discharged from hospital if they are taking codeine phospate 30mg 2 tablets every 6 hrs max of 8 tabs 24hr but accidently takes next dose after only 4 hrs but has no real adverse effects could it be a danger?

No, for as long as staying on the maximum allowed dose of 8 pills/day. If pain no longer controlled or pain relief effects not lasting more than 4 h let your prescriber know.
