An LPN cannot prescribe medication. You should consult an endocrinologist and monitor your TSH level.

Dosing Synthroid is largely based on TSH level, so a dose increase with a TSH of 7.1 is likely reasonable. In some brands of levothyroxine (the medication in Synthroid), there are 300 mcg pills, and some patients do take doses that high or higher. The clinician will likely want to recheck the TSH level in 6-8 weeks to monitor if the medication is working well.
