Significant head trauma while on anticoagulants is definitely a danger. The direct acting anticoagulants liklike apixaban are much less apt to cause brain bleeds spontaneously.

Occult bleeding (externally and internally) is rare but the risk slightly differs depending which anticoagulant you are on. According to this study, "overall, apixaban, and dabigatran, but not rivaroxaban, were associated with less total bleeding and death than warfarin in patients with heart failure and atrial fibrillation" (https://www.ahajournals.org/doi/10.1161/CIRCOUTCOMES.120.007230).
