I assume he doesn't have diabetes or is taking any medications that could cause these symptoms. I assume he has no mental or psychiatric issues. HAS he been referred to and evaluated by a neurologist to determine whether he has a neurogenic cause of loss of bladder and rectal control? Psychiatric evaluation needed? Discuss these questions with his MD.

First the cause of both problems has to be determined by exam and testing before any treatment can be given. He needs to see someone at an academic center for that.Good luck
