This looks like an add on to some prior post. None of your posts on this site are linked nor can we search for them. Your insurance contract describes what is covered. Some procedures/tests/medications/etc. require pre-approval by insurers or even a review of your past medical records before they are authorized. The help desk at your insurer or the HR department if a work supplied policy may know
