Why do some doctors order and consider fibrinogen as a risk factor for heart attack and stroke and not d- dimer? which of these 2 tests is a better risk factor in predicting future (5 months or more down the road)heart attacks, strokes, dvt and, pe?

In creased fibrinogen levels can increase formation of blood clots, which can "predict" increased frequency of cardiac disease. A "D-dimer" test is an acute phase test to assist in "ruling out" present symptoms of pulmonary emboli, and hence is not a "predictor" of future events.
