Strokes and heart attacks happen at a moments notice. a polst form, therefore, is not signed off by a physician in the er when patient has lost decision making ability. is it ever possible that a polst is signed prior while not diagnosed yet?

Yes, ideally POLST or MOLST forms are to be signed early with the primary care doc as they should represent the patients true wishes when it comes to intensity of care at end of life. Many times such orders are reversed in ER, due to the eternal dilemma of keeping the patient alive at all cost by prolonging their suffering.
