You need to lose weight by all means. There is no reason to have a big heart after a pneumonia but I do think you have big heart, mainly the right ventricle, because of high lung pressure (pulmonary hypertension) and pneumonia is blamed but more likely due to obesity. Your cough is either due to post nasal drainage due to sinus versus reflux (GERD), you should be checked by an experienced ENT.
