High glucose in the urine does not cause sweet smelling urine and likely glucose does not appear in the urine in prediabetic condition. Glucose only appears in the urine when blood sugar is above 180. Sweet smelling urine can be present when ketone bodies, acetone is one of them, are high and this symptom occurs in diabetic ketoacidosis, a severe complication of uncontrolled diabetes mellitus.
