Incision and biopsy is many times diagnostic when a lymph node is enlarged and overall clinical leads point towards significant pathology. Otherwise there are many characteristics of enlarged lymph nodes that point toward benign pathology and they can be be followed up clinically.

For a definitive diagnosis, the lymph node should be removed in its entirety and submitted to a pathologist for examination.
