I am slim (46kg). if genetic inherited lipoprotein (a) is very high (159 mg/dl), cholesterol 214, ldl 146 and hdl 62, is there a serious risk of health issues? can lifestyle improve it or only medication? should i take meds at 40 already?

The issue you describe is important and complicated enough that you should consult a cardiologist for making sure that your cardiovascular health is addressed better than is feasible in this forum. It is good that you are addressing the issue proactively. Wish you good health!
