There is nothing to point out PCOS with these results. However, you have borderline hypothyroidism if repeated TSH is still above 4.5 and this condition can affect your menses and weight. T4 is in the low side of normal range so a T4 total should be done when TSH is rechecked. Make sure you do not take significant amount of biotin because this vitamin can affect TSH level with certain lab methods.

All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, your TSH result is consistent with hypothyroidism and you should talk to your doctor about replacement medication. Wish you good health!
