Your first step is to call the doctor's office to complain and to emphasize that you specifically refused the test. Although the lab bill usually comes from a separate company, they will normally honor the doctor's request to remove the charge. You may contact the lab directly but that is less likely to be successful though they will usually at least discount the fee for self-pay.

You can talk to her. If that does not work send a registered letter explaining the situation and that you will not pay for those reasons. This should work if not you might need to contact the county and /or state society. Or you can consult a lawyer.
