My friend got an mri after a concussion. her report said "localized paramedian encephalomalacia". googled this, and it seems like something requiring immediate attention, but her followup isn't for another five months. is that too long to wait?

This would be a question for her personal physician. If she has concerns about the treatment she received at the ER, her Dr (who has all her history, etc) would be the next step. There are many cases where a patient with mild Localized Paramedian Encephalomalacia is sent home with a list of symptoms to watch for. There is usually no treatment for LPE in a non symptomatic patient.

The interpretation is consistent with prior pathology, and has likely been present long before the recent concussion. Since this has caused anxiety on your part, contact the radiologist and discuss the significance of the findings.
