On a culminating pregnancy report, i found this note next to an early date: suspect vanishing twin. single iup at this time this is the first i have seen or heard this. would my doctor disclose if i had been pregnant with twins?

I would ask the doc. The language says "suspect" not confirmed. There is no way to know what the suspicion was based upon with this minimal information.
