Had surgery for a chiari malformation 4 years ago. yesterday i decided to ride a roller coaster ride (not a good idea as i now know.) now i have extreme pain in the back of my head into my shoulders. what could be the problem?

The most likely issue is a cervical strain secondary to the acceleration-deceleration of the roller coaster affecting an area of post-operative scarring. A few sessions of physical therapy can help. Just to be on safe side, notify your prior neurosurgeon and have this more fully evaluated.
