Orthopaedic Surgeon: Make an appointment with a board certified orthopaedic surgeon, preferably one who specializes in shoulders. If you have and are not satisfied, get a second opinion. Any doctor that knows his/her specialty should be offended by this.

Time for consult: If you have not already done so, i would suggest a consultation with an orthopaedic surgeon and a MRI of your shoulder. The length of your symptoms suggests a persistent shoulder problem that needs to be evaluated fully.
