Seek treatment for your neck and shoulder pain. Musculoskeletal pain is responsive to many approaches and therapies. A physiatrist is experienced in this.

Chiari malformation: Based on your age of 26 it is unlikely that the chiari will progress, if you were a child then there would be more of a concern for progression.

Can be assoc w/ pain: Pain can be associated with chiari malformation. It is important to exclude other causes of cerebellar tonsillar herniation, such as a mass. This would be with contrast MRI brain and cervical spine. Chiari malformation can have syrinx of the cord, fluid filling the central portion of the cord. In setting of pain, this should excluded, with MRI spine to include contrast.
