It is not a routine test for most patients. Doctor only orders TTE when a heart condition is suspected by medical history or physical exam, e.g. sign and symptom of heart failure, coronary heart disease or valvular heart disorder.

Transthoracic echo is not a routine test, but used as part of the workup for unexplained symptoms such as shortness of breath or leg swelling, to name a few, and in order to determine the status of the heart and its possible involvement.
