Severe back and neck pain for over 5 years. mri, x-ray and ct scan are all clear. pain started in my lower back. radiates to upper back and neck. lower back is no longer as severe. upper back and neck usually start at night lvl9. now what?

Hope you had a good clinical exam and your MD had a tentative diagnosis before these tests. It is difficult to say what is causing it without exam. It could be muscular or fasciitis. Try some physiotherapy and heat. If not better pl. see a Ortho or neurologist. Once the reason is found you should be OK
