Not how it works. Your doc or pediatrician who has knowledge and legal documentation of your chronic medical conditions would be the best and only resource to have a medical exemption written in your behalf.

The state medical board can restrict or cancel a docs license for providing any note that is not justified by evidence kept in the record of any visit. The court has blocked the OSHA mandate which OSHA also put on hold. If you live in an "AT Will" employment state your employer can set whatever requirements they want for continued employment and fire you at will.

Why do you not want to get vaccinated? You have not provided any cogent reason for not taking. More than 4 Billion people have taken the vaccination without any problem. Your imagined ill effects from vaccination are far rarer than your chance of dying from snake bite or lightening strike. Take the vaccine.
