Yes, it can if you have high grade varicocele bilaterally and they are not fixed. However, bilateral varicocele is not common.

In some people under certain conditions. Just because there is a varicocele does not mean infertility. It can cause lower sperm counts which is but one cause of infertility. There are many people who have varicoceles who are not infertile. In most people successful surgery restores sperm counts enhancing fertility. So the short answer is -No
