Sharp pain in right temple, spread around my head on the r side. can make me shout out ouch. 2-3 times a day, 2-3 seconds at max! had an mri 1 year ago and came back clear and normal! sometimes ear pain. hypochondriac worried about aneurysm or bleed?

No, severe headache is the most common initial symptom of intracranial aneurysmal bleed. The pain you describe seems consistent with trigeminal neuralgia. You should see an internist or a neurologist for a therapeutic diagnosis because MRI was negative.

Your history/ description and duration is entirely inconsistent with either bleeding or aneurysm presence. Although some migraine variants can cause icepick pains, the focal consistent location is more consistent with a V-1 trigeminal neuralgia. As recommended by Dr Le, a neurologist is the next step to address this.
